During the course of questing, items may break from time to time due to their individual break percentage. Chances are that the item equipped was not an ideal fit for that specific hero. Items that are either too high or low in level compared to the level of your hero are more prone to being broken.
You can always gauge the likelihood of an item being broken by referencing the break percentage as you equip the item onto the hero. Be sure to check your items frequently as the break percentage changes every time your hero levels up.
You can choose to repair items after a quest using gems.
Things to remember and tips:
1. Each item has its own individual role. Sometimes, this can lead to the confusion that items break at higher percentages than listed since different item types may break on consequent quests.
2. Each quest is a new roll for each item.
3. The closer to the Hero’s level and better the quality of the item, the lower the break chance. Epic, Legendary, and Mythic items will be unbreakable for at least one level to that of the hero.
